Cycling for Change Registration fees are as follows:
One-day: $100

4 Day and 7-Day Rider Information

Cycling for Change 4-Day and 7-Day Riders must complete the following Registration Forms:

Cycling for Change 2012 Registration form
Health Information Form
Authorization for Release of Information—Driver Information
Cycling for Change Participant Agreement
Family Care Safety Registry
Investigative Consumer Report Disclosure

Cycle for Change Cycling for Change 4-Day and 7-Day cyclists must provide copies of the following:

Driver’s License
Health Insurance Form
Social Security Card

Please mail the completed Registration Forms along with a copy of your driver’s license, health insurance card, and Social Security card to:
